Below you will find the application form to use to apply for low cost telephone service through a well known Internet Service Carrier which will allow you to make unlimited length phone calls in the Eastern US for a maximum charge of 99 cents (10 cents a minute up to 10 minutes and then free from then on), and unlimited length phone calls to the rest of the USA for a maximum charge of $1.99 (10 cents a minute for the first 20 minutes).   Alternately, you can choose their other plan which allows you to make phone calls to anywhere in the US (including within your state - Sprint or MCI can't match that price!) for only 4.9 cents a minute.  Unfortunately you can't have both.
